Initial consultation:  

The initial appointment can go up to 90 minutes and it will be a virtual appointment using Zoom Telehealth.  

 

 

 

1.) Please see the Telehealth appointment instructions here.

2.) Prior to your first appointment, you will need to fill out and send in several forms.  You will find the forms further down on this page. 

 

3.) Please fill out/ email the completed documents 24 hours prior to your appointment in order to be seen. 

 

Follow up appointments:  

Follow-up appointments are 30-45 minutes long and are scheduled every four to six weeks.  I welcome all your questions during follow up appointments, please keep a running list. 

 

More Info: I am not on call. I will do my best to return your emails within 24-48 during my regular office hours.  For any and all medical concerns please see your medical doctor or an urgent care provider if necessary. I typically take Friday to Sundays off.

 

Insurance:

I do not work with insurance companies but I can provide you with a receipt upon request.

Fees:   

The fee for the initial appointment is $200 and the 4-6 week follow up appointments are $75.

Acute illness are $50. (Acute illness packages can be worked out for whole families who are sick with a bug.) 

 

Payments:   

The fees for consultation are payable by PayPal, Venmo or contact me for an address to send a cashiers check. All payments must be made prior to the appointment.
